Enrollment process

Expedite your enrollment in a few simple steps!


Step 1:

Enter information about you and your child using the online interest form, then print and complete an enrollment packet in English/Spanish or Chinese.

Step 2:

Gather and upload (via the online interest form) the following documentation:
  • Birth certificates for ALL children in the household;
  • Current (within 12 months) physical exam and TB test with results for enrolling child;
  • Up-to-date immunization record for enrolling child;
  • Working parents: All paycheck stubs received in the last 30 days (for set work schedule) to four months (for variable work schedule);
  • Parents in school/training: Current school enrollment/training schedule;
  • Proof of any other income: TANF, unemployment, disability, child support, etc.
