Expedite your enrollment in a few simple steps!
Enter information about you and your child HERE, then print and complete an enrollment packet in English, Spanish, or Chinese.
Gather and upload (via Online Interest Form) the following documentation:
- Birth certificates for ALL children in the household;
- Current (within 12 months) physical exam and TB test with results for enrolling child;
- Up-to-date immunization record for enrolling child;
- Working parents: All paycheck stubs received in the last 30 days (for set work schedule) to four months (for variable work schedule);
- Parents in school/training: Current school enrollment/training schedule;
- Proof of any other income: TANF, unemployment, disability, child support, etc.
