Upload any/all of the following Express Enrollment documents if you are ready to submit enrollment documentation:

  • Completed enrollment packet;
  • Birth certificates for ALL children in the household;
  • Current (within 12 months) physical exam and TB test with results for enrolling child;
  • Up-to-date immunization record for enrolling child;
  • Working parents: all paycheck stubs received in the previous month (for set work schedule) to 12 months (for variable work schedule);
  • Parents in school/training: current school enrollment/training schedule;
  • Proof of any other income: TANF, unemployment, disability, child support, etc..
